Instructions:

• Cut letter size paper in half down to 8.5x5.5 inches.
• Download PDF file
• Open PDF file in Adobe Reader or Preview (on Mac).
• Click text “type name here”
• Type in the name of your choice. 
• Click Save to save file.
• Open Printer.
• Change paper size settings to 8.5x5.5 inches.
• Print as many sheets as you need!
